PilotsarguedasFirstAirighthurtledtowarddisasteratResoluteBayTransportationSafety7TuesdayApril12014JoinusincelebratinglifelonglearningandalladultlearnersintheNorthwestTerritoriesduringInternationalAdultLearnersWeekinCanadaKnowledgeisapowerfulinstrumentforchangeandlifelonglearninghelpsustoleadhealthyproductivelives.Learningateverystageoflifeopensdoorsthatleadtonewopportunitiesandself-fulllment.TherearemanyprogramsinplaceacrosstheNorthwestTerritoriesdesignedtosupportalladultlearners.Whetheryouwanttoattainyourhighschooldiplomaoradegreemoveupinyoureldorexpandyourcreativesidewecanhelpyoureachyourgoals.Visitwww.ece.gov.nt.catolearnmore.March29April62014Hon.JacksonLaertyMinisterEducationCultureandEmploymentMasiPhotoAbdallahhByJACKDANYLCHUKHumanerrorweatherandequipmentfailurewereamong18factorsthatcon-tributedtothefatalcrashofaFirstAirBoeing737inAugust2011accordingtoaTransportationSafetyBoardreportreleasedlastweek.Threepeoplesurvivedthecrashthatkilled12includingthepilotandco-pilotwhodis-agreedoverwhattodointhenalminutesasFlight6560attemptedtolandatResoluteBayonarainyfoggyday.IdontlikethisFirstOfcerDavidHaretoldCaptainBlairRutherfordasecondbeforetheplanesgroundproximitywarn-ingsystemsounded.Inthenextinstanttheplanecrashedstrew-ingdebrisonalowhillakilo-metrefromtheairport.Flight6560wastheonlyBoeing737intheFirstAireetnotequippedwithamoreadvancedterrainawarenessandwarningsystemTAWSwhichbecamemandatoryforcommercialpassengerair-craftayearaftertheReso-luteBaycrash.TAWSwouldhaveenhancedthecrewsawarenessoftheirsituationandgiventhemmoretimetoreactthereportsaidbutleadinvestigatorBrianMacDonaldtoldTheJournalthatitsnotcertainitwouldhavepreventedthecrashthatByMARIACHURCHFortGoodHoperesi-dentswillneedtimeandcommunitysupporttodealwiththedeathofCharlotteLaffertyamotherofthreewhowasmurderedinthecommunitylastweekbuttheyhavedealtwithtragedybeforeandwilldoitagainsaysthechief.GregLaboucanchiefofFortGoodHopetoldTheJournalthecommunitywillbesupportingallwhohavebeenaffectedbythetragedywhichincludesthefamilyandfriendsofthe17year-oldchargedwiththemurderofLafferty.TheyarealsovictimsinallthisaswellandwehavenothingbutsupportforthemLaboucansaid.LaffertywasfounddeadintheearlymorningofMondayMar.24neartheseniorscomplex.RCMPwerelateronsceneandchargedayoungmanfromthecommunitywithrst-degreemurder.Thisissomethingthatisgoingtotakealonglongtimeifevertogetover.ItsjustahorribletragedyandonethatyouknowwehaveFortGoodHopelooksforhealingaftermurderLeadersconcernedwithRCMPresponsetimewascharacterizedasacon-trolledightintoterrain.ThisaccidentwastheproductofacomplexseriesofeventsallofthemlininguptogetherMacDonaldsaid.Butwhatultimatelytiedallthesethingstogetherwasthatastheightprogressedeachpilotdevelopedadifferentunderstandingofthesitua-tionandtheywereunabletoreconcilethatdifference.TroublebeganforFlight6560whenRutherfordbeganthedescentintoResoluteBay.Theanglewastoosteepandtheplanewastravellingtoofastasitapproachedtherunwayabrisktailwindwaspushingitoffcourseandtowardthehillwhereitcrashed.Theplanescompasswas17degreesoutandtheauto-pilotwasaccidentallydisen-gagedas6560madeitsnalapproachaddingtoconfu-sionandtensionontheightdeckthatwasevidentintheverbalexchangesbetweenRutherfordandHare.Withinsecondsafterthatnalturntheco-pilotreal-izedtheplanewasoffcourseandrepeatedlytoldthepilotremindinghimaboutthelargehilltotherightoftherunway.Rutherfordrepliedthattheautopilotwasworkingne.Puzzledastowhytheplanesnavigationalinstru-mentswerentliningupwithground-basedsystemsHareaskediftheyddonesomethingwrong.Fivesecondslaterhesuggestedtheypullupandgoaroundforanotherapproach.Rutherfordfullyfocusedonlandingtheplaneandonguringoutwhyhisinstru-mentsweregivingconfusingreadingsrefused.Itislikelythatthecaptaindidnotfullycomprehendin-formationthatindicatedthathisoriginalplanwasnolon-gerviablethereportsaid.Lessthan10secondsafterrstsuggestingtheypullupHareaskedagainpointingoutthattheplanewasntcon-guredforalanding.There-portsuggestedRutherfordislikelytohaveunderstoodtheremarkasarequesttopre-paretheplaneforlanding.Thecaptainsmentalmodelwaslikelythattheap-proachandlandingcouldbesalvagedandtheco-pilotsmentalmodelwasalmostcertainlythattherewassig-nicantrisktothesafetyofightandthatago-aroundwasrequired.Thesedivergentmentalmodelscompromisedthepilotsabilitytocommu-nicateandworktogetherthereportsaid.Thereportmadeonemainrecommendationimprovingcommunicationbetweencrewmembersonallplanes.BlairandRutherfordhadreceivedoutdatedcrewmanagementtraininginatwo-daycoursethatwascompressedintofourhours.Therstofcerssug-gestionswerentcompellingenoughtoalterthecaptainsmindsetandtherstofcerlikelyfeltinhibitedfromtakingcontroloftheaircraftfromthecaptainboardmemberKathyFoxtoldre-portersinOttawawherethereportwasreleased.Crewresourcemanage-mentissupposedtohelpightcrewsinexactlythesekindsofsituations.FoxsaidTransportCan-adaisupdatingitstrainingbutshewarnedtherewillbegapsunlessallairlinesarerequiredtoapplythestan-dardsonadailybasis.Ablizzardoflawsuitsfol-lowedinthewakeofthecrashnamingFirstAirNavCanadaandthedepartmentofNationalDefenceasdefen-dants.CanadianForcessov-eriegntyOperationNanookwasinfullswinginResoluteBayandtherewasspecula-tionthatitmighthaveinter-feredwithFlight6560butitwasnotamongthefactorsthatinvestigatorsincluded.Theboardearlierrevealedthatanotherplanewasinthesameareaatthetimeandposedtheriskofamid-aircollision.MacDonaldsaidfurtherin-vestigationshowedbothwereawareofeachotherandtheotheraircraftlandedsafely.abigemptyholerightnowinourcommunityLabou-cansaid.ThecommunityheldacandlelightvigilforLaffertyMondaynightorganizedhoursafternewscirculatedofthemurderandcharges.Morethan40peoplewereinattendanceincludingthefamilyofthevictim.AtraumateamhasbeenbroughtintothecommunityfromTulitatoprovidegriefcounselling.Reportsthatalcoholwasafactorintheas-saultpromptedthechiefandcounciltorequestanemergency30-daybanonimportingliquorintothecommunity.RCMPresponsetimeaconcernFortGoodHopechiefandcouncilmetwithRCMPof-ficersTuesdaytodiscussresidentsconcernsaboutresponsetimeafterrumourscirculatedthatthebodywasleftinthestreetformorethananhourbeforeRCMPshoweduptoinvestigate.SomepeopleweresayingittookoveranhourforthemtorespondbutaccordingtoRCMPrecordsitwasallwithin15minutesfromthetimetheinitialcallhap-penedLaboucansaid.ThecommunityisgenerallycontentwiththeperformanceofRCMPofcersinFortGoodHopehesaidbutthemeet-ingwasanimportantdebrieffollowingtherattlingtragedy.Laboucansaidtherearestillsomeconcernsthatemergencycallsmadeout-sideoftheFortGoodHopeofficehoursgothroughYellowknifedispatcherswhichcanslowdownre-sponsetime.ItdelaysthingsbecausetherearesomanyquestionsandanswerstheyaretryingtogetandsometimeswefeelthattheyaretheonesdeterminingiftheyshouldcalltheRCMPoriftheyshouldnthesaid.Itcangetfrustratingespeciallywithanemergencysuchasthisonetogothroughallofthat.Thatjusttakesanotherveminutesawayfromtheresponsetime.RCMPofcersinthecom-munitysuggestedinthefuturethatresidentsinformthedis-patcheroftheemergencyaskthemtocontacttheFortGoodHopeofcersthenleavetheirnameandnumbertoavoidlengthyquestioning.HumanerrorweatherandequipmentfailurecombinedtocausethedeadlyFirstAircrashinResoluteBayin2011accordingtotheTransportationSafetyBoard.JusticeHomicide
